  1. Please complete this RMA FORM before any RMA# can be issued. After product problems have been reviewed, we will contact you and issue a RMA number for you.

  2. All RMA products must be returned within 45 days of receipt of an RMA number otherwise the RMA number will be voided and the package will be refused. Returned products must be freight prepaid, secured in safe packaging method and please clearly mark RMA number on outside of package and enclose a copy of this RMA FORM. Damage or loss of goods during shipment is the sole responsibility of the customer.
